My first day ice breaker

9 Aug
On the first day of school I usually do an ice breaker activity where students roam around and have other students sign somewhere on their paper.
I decided to make this a Google Form because they will not be able to roam about the room.   The first day may turn out to be pretty difficult, this may move to the second day.  This one is in English for first year.  I have one in Spanish for second year.
My plan is to have students answer the form (I am assuming they will have the Chromebooks the first day and will get them on my Google Classroom right away.).  When they are finished you can go to the form and click on the Responses tab.  Project this on the screen and see what the answers were in the pie charts that are generated.  Students will see what they have in common.  As we go through each question I will ask, for example, who answered yes to this question (for example, Do you have more than 4 siblings).  Then pick a couple of students to elaborate.  I will learn a little about some or all of them.
The trick to my form is that after going over the answers I tell the students that all of the prompts are true about me, then ask if they have any questions about me after learning what they have from the form.
Click below if you would like to make a copy of the form I am using.  You will be able to change the questions to anything you like.

Hey everyone!  I just learned that you will not be able to make a copy of this form from your phone, you must access it from a computer.  Sorry about the confusion.
